Set in 1967, the story follows John and Mia Form, a young couple expecting their first child. John gifts Mia a rare, beautiful vintage porcelain doll for her collection. However, their domestic bliss is shattered when members of a satanic cult break into their home. During the violent encounter, a cultist named Annabelle Higgins dies while holding the doll, her blood seeping into its porcelain eye and transferring a dark spirit into it.

The couple’s peaceful lives are violently upended when their neighbors, the Higgins family, are murdered in the middle of the night. The killers are revealed to be the Higgins' estranged daughter, Annabelle, and her boyfriend, both members of a murderous satanic cult. When police arrive at the scene, the boyfriend is shot dead, and Annabelle Higgins commits suicide inside the Forms' nursery while holding the newly gifted porcelain doll. A drop of her blood seeps into the doll's eye, anchoring a demonic entity to the object.
